I would give the 4 cc every 12 hours,I would continue treatment for a full 24 hours after he is 100% better, when he is fully recovered, re boost his C D&T 10 days after the last tetanus antitoxin shot for long term protection...be sure to give him the tetanus antitoxin before messing with the testes area ...protect him from further toxin release.. Check the banding to be sure there is no infection noticable outwordly to tend to..

Ok so a burdizzo clamp was used? I would go ahead and treat with Penciliin for 5 days incase internal infection. Check his temp see if that is with in normal range as well...(101.5-103.5)
keep up the B complex...

IF he can swallow well you can soak alfalfa pellets in water and dentch him using a turkey baster to help keep food in him. If he can chew, you can Roll hay into tiny balls and stuff it in his mouth. Tetanus is hard to come back from but keep at it...if you can keep food in him that will help...here is an electrolyte recipe that i good to drench...ad good stuff in it for him...
Keep up the B complex every 4-6 hours...Did you start Penicillin? if not start that too 1 cc per 20# Sub Q...be sure not to get it in a vien..Pull back on the plunger..see blood, re adjust and try again, no blood is good to go...

Homemade Electrolytes
A half gallon of hot water
2-6 Tablespoons of Unsulphured Blackstrap Molasses
1-2 Tablespoons of Either Sea Salt, Epsom Salt, Baking Soda or Table Salt.
1 cup of Apple Cider Vinegar

Mix well and drench or let them drink it

I fed a goat with polio by rolling tiny balls of hay and sticking it in his mouth..he could not eat on his own...you can visit my web sight and click Dozers story on the Goat drama link...
